What it is, How it works

Hair samples are straightforward to collect, relatively challenging to tamper with, and simple to transport.

A sample measuring 1.5 inches in length with approximately 200 hair strands (roughly the size of a #2 pencil) nearest to the scalp provides 100mg, the optimal amount for screening and confirmation. For tests involving EtG, supplements, and/or more than 10 panels, 150mg is advisable. Specimens should be weighed using a jeweler’s scale. If scalp hair is not available, a comparable quantity of body hair can be collected. Head hair strictly refers to scalp hair, whereas body hair encompasses all other types (such as facial and axillary hair).

Process Overview

The laboratory processing of a drug test result primarily involves four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the sample's initial processing into the lab’s system. This step ensures the sample was securely sealed and mailed correctly, assigns a unique LAN (Laboratory Accessioning Number), and completes any missing data entry not covered by an electronic chain of custody system.

Screening constitutes an initial rapid assessment for abused drugs. Although Screening is a viable way to assess drug usage on the majority of samples, a positive result requires Confirmation to be court-admissible. Any sample that screens positive will need a secondary Confirmation test.

Where a sample tests positive in Screening, additional hair from the original sample is extracted for further Extraction. This phase involves isolating drugs from hair at much lower concentrations compared to alternative methods (e.g. urine or oral fluid), making hair drug screens one of the most challenging techniques to conduct.

Confirmation for any positive screening outcome is accomplished via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed prior to Confirmation as necessary. The entire lab process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
