For EtG, additional testing, and/or panels exceeding 10, 150mg of the sample is suggested. It is advisable to measure the sample using a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be used. References to head hair imply only scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result comprises Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the introductory step of a sample’s entry into a lab’s system. This involves confirming the integrity of the sample's seal and proper shipping, allocating a LAN (Laboratory Accessioning Number) randomly, and completing outstanding data entry absent from an electronic chain of custody system.

Screening is an initial rapid assessment for drugs of abuse. Screening economically excludes drug use in most samples, but a positive result in screening necessitates confirmation for court admissibility. Any samples marked as presumptively positive during Screening need further confirmation.

For samples with a presumptive positive in the Screening phase, more hair is extracted for the Extraction process. During Extraction, drugs are isolated from hair at much lower levels than in other methods (e.g., urine or oral fluid), making hair drug testing particularly complex.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples undergo washing before confirmation if required. The full lab process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.
