Process Overview
The laboratory processing of a drug test result comprises four primary stages: Accessioning, Screening, Extraction, and Confirmation.
During Accessioning, the sample undergoes initial processing in the laboratory system. This involves confirming the sample was securely sealed and dispatched, assigning it a unique LAN (Laboratory Accessioning Number), and completing any other data entry required that is not covered by an electronic chain of custody system.
Screening serves as an initial rapid assessment for drug substances. Although Screening is a cost-effective means to discard the possibility of drug use for most samples, a positive result must be confirmed for legal validity. Samples that show a provisional positive in Screening need further confirmation.
When a sample shows a provisional positive at the Screening phase, additional hair is extracted from the original sample for further Extraction. At this stage, substances are extracted from hair at a significantly lower concentration compared to other techniques (e.g., urine or oral fluid), highlighting the complexity of hair drug testing.
Confirmation of any positive screening is done using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, possibly positive samples are washed if necessary. The complete laboratory process, from Accessioning to Confirmation, is vetted under the CAP (College of American Pathologists) Hair standards as well as accreditation per ISO / IEC 17025 protocols.
