A 1.5-inch lock of roughly 200 hair strands (comparable to the size of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal sample for analysis and validation. For EtG, additional elements, and/or screenings exceeding 10 panels, it is advised to have 150mg of the sample. We suggest weighing the sample using a jeweler's scale. If scalp hair is not available, a comparable quantity of body hair can be used. Head hair specifically refers to scalp hair. Body hair encompasses all other hair types (e.g., facial, axillary).

Process Overview

The primary stages in the lab procedure for drug test analysis are Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the introductory handling of a sample within a lab system. This process comprises ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data submissions not included by an electronic custody chain.

Screening includes an initial rapid check for substance abuse. Although Screening is cost-efficient to eliminate drug usage in most samples, a positive screen needs confirmation to be valid in legal settings. All samples suggesting positivity in Screening require an additional confirmation.

If a sample tests positive in Screening, additional hair is extracted from the initial sample for the Extraction phase. In this stage, drugs are isolated from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluids), making hair drug screening a more complex methodology.

Confirmation of any positive screening outcomes is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positivity are cleaned before confirmation if necessary. The full laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair criteria and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
