When collected near the scalp, hair can offer a detection period of up to around three months for drugs and alcohol. Hair is easy to gather, relatively hard to tamper with, and convenient to send.

A 1.5-inch sample comprising approximately 200 hair strands (roughly the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample for both screening and confirmation processes. For EtG, additional analyses, and/or panels exceeding 10 tests, a 150mg specimen is advisable. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair isn't accessible, an equivalent quantity of body hair may be used. When mentioning head hair, it is specifically scalp hair. Body hair includes other types like facial or axillary hair.

Process Overview

The primary steps in the lab processing of a drug test result involve Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the initial stage of processing a sample into a lab’s system. This step ensures the sample was correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and concludes any data entry not already available through an electronic chain of custody system.

Screening provides an initial rapid test for drugs of abuse. While Screening is cost-efficient for eliminating drug use in most samples, a positive result must be verified for court acceptance. Samples showing presumptive positives in Screening require secondary confirmation.

If Screening indicates a presumptive positive, additional hair is extracted from the initial sample and prepped for the Extraction stage. Here, drugs are drawn from hair at a significantly lower concentration than other methods (like urine or oral fluid), making hair drug screening the most challenging to execute.

Confirmation of positive results is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are washed before confirmation when necessary. The entire laboratory workflow, from Accessioning to Confirmation, aligns with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
